What Is a Midline IV and What Is It Used For?

When patients require intravenous (IV) therapy for more than a few days but do not need a central line, a midline IV catheter can be an excellent option. Midlines provide reliable vascular access while offering greater comfort and fewer needle sticks for patients.

What Is a Midline IV?

A midline IV is a specialized catheter that is inserted into a vein in the upper arm, typically using ultrasound guidance. The catheter is longer than a standard peripheral IV but shorter than a peripherally inserted central catheter (PICC) line.

Unlike a PICC line, the tip of a midline catheter remains in a peripheral vein and does not extend into the large veins near the heart. Because of its placement, a midline can provide stable venous access for patients who require treatment beyond the typical lifespan of a standard IV.

Midline catheters can generally remain in place for up to four weeks, depending on the patient's condition, the type of catheter used, and facility protocols.

  • Certain chemotherapy agents
  • Solutions with extreme pH levels
  • Highly concentrated or high-osmolarity medications
  • Long-term therapies requiring central venous access

In these situations, a PICC line or another form of central venous access may be more appropriate.

The Insertion Process

Midline placement is typically performed by a specially trained nurse or vascular access specialist. Using ultrasound guidance and sterile technique, the catheter is inserted into a vein in the upper arm and secured in place. The procedure usually takes less than 30 minutes and can often be completed at the bedside.
